Please also provide your FULL NAME in English and Chinese in the registration form below. You are required to wear white garment during the ceremony.) 皈依儀式。欲皈依者須預先於網上報名，並於二零一七年四月二十八日前提交近照兩張 (30毫米 X 20毫米) 至本中心 (香港德輔道西410號太平洋廣場28樓)，以便中心製作皈依證，亦煩請於下列表格提供中英文全名。皈依者須於當日穿著白色衣服，並將獲發法名。)
